Hippocratic oath : The definition of maximum

If you’re watching a TV, an Olympic channel, or simply go to YouTube and searching for Usain Bolt performance in Olympic. You’ll see an incredible speed of running. A speed of light.  A clear win. But, what if I ask you how fast do Usain Bolt can run? 10.38 m/s? If it’s that so, if people already know how fast Usain Bolt can run, then why do we always need to measure his maximum speed  every time he perform in Olympic? The answer is simply because that word maximum is relative or the event of maximum performance is relative.

If you ask Usain Bolt to run without shoes on a muddy track while a hundred kilogram of luggage tied tightly with a chain in his left hand, I’m sure that he will not even reach 3 m/s speed, which in this case even my little brother can beat his record (gold medal?). This thing is what happening today to doctor in Indonesia. A lot of people claim that some Indonesian doctors are not professional, some of Indonesian doctors do the malpractice, some of them do not act maximally. Yes, hippocratic oath suggests the doctor do the maximum effort for their action. But, do the Indonesian doctor did not act on its maximum effort? To answer such a question, we should analyze first, what kind of track that they are going through? A muddy one? Or a nice one?

Indonesian doctors face a lot of problems, insufficient equipments is one of it. To diagnose a disease, we need the help of equipment, for example to diagnose parasite in our body. Indonesia don’t have that kind of serological tools to diagnose it like Thailand or any other countries which lead to a late diagnose for some case of parasitic infection. Just like Usian Bolt without shoes whose can’t run in a maximum speed. Some bad habit from Indonesian people also affects the effectiveness of the treatment. Bad habits of consuming drugs without the instruction of the doctor make some disease become resistant to the drugs. For example the drugs for malaria in papua which become resistant. This situation made the track that doctor going through become harder. The habit of Indonesian people which come to doctor when his/her disease is severe is also making the situation that doctor have to face is harder. The greater the disease, the greater the effort that it takes, and the greater the risk for the patient that make the chance of a fail treatment occur which somehow makes the doctor seems not doing it best effort. Yes it’s seems so. It’s about point of view.

If we are going back to the case of Usain Bolt. Giving him a best situation just like when he runs in the Olympic track. Do the speeds of Usain Bolt is absolutely 10.38 m/s for everyone whose measure it? Albert Einstein says no to that. He says it’s relative toward who saw it. If it’s a kid who’s riding a rocket in the same direction with Bolt with a speed of 10.38 m/s, he can say that bolt move with 0 m/s speed. This case of a relative point of view is also happening to the case above. People who don’t know about the risk of medicine seem to see the doctor always fail. For example the case of cancer and chemotherapy which give a really severe side effect. People sometime see that doctor do a malpractice simply because of the side effect of chemotherapy. In fact, that doctor has been calculating that the life expectancy of his patient that he may extent is far more greater than letting that cancer to grow up and can kill that patient one year after, even if it that cancer give no symptoms at the presence time.

Another curve that people also forget to consider that doctor itself is also a human which also have some problems that can reduce its performance. Seeing all that conditions, we can see that a maximum performance that the doctor in Indonesia can do is restricted by some factors. Which the presence performance might be its maximum performance regarding to those restrictions. I beieve that they have done their best. I hope for the best.

nb : this post is presented to you guys in order to have a better understanding upon the situation of doctor especially in developing country. I mean, do you always get a perfect score of 100 in math while the equation of math like 2 +2 equal to 4 or other equations had never changed, you don’t right? Compare to a medical discipline where the knowledge itself is always changing every day, so do the disease and the symptoms that it presented. That’s why sometime it’s called as an art, an art and skill to diagnose a disease. But with this exception, I’m not saying that I’m supporting the doctors who do failure nor want them to be exist.I still believe in the intelligent doctor. I believe in inspaning verbintenis. A maximum performance by making every single factor in its maximum function.
